1) You will need to tailor the letter and Declaration of Status for you.
2) The Acknowledgement page needs to be printed for both the Notice of Liability and Declaration of Status. Therefore, you will need to print it twice. This is the form you take to get notarized by the De Facto. Perhaps your bank offers free notaries as mine does.
3) The Notice of Liability is applicable to many circumstances, such as the school your off spring will be attending or for your employer. This applies to high school, college, universities, grade school and preschool

thank you. I use Affinity for my documents. I've tried to convert into an editable format and it is not easy with Affinity. Can you copy and paste it into your software for you to edit? As for fee schedule, I used Anna's website https://tasa.americanstatenationals.org/fee-schedule/ for the template.

I didn't think about copying and pasting the text. It actually worked! I was able to copy the text to a Word document, I can then make any changes I need to it. Thank you so much for responding. Thanks for the link to the fee schedule. I will edit and use that also.
