I wrote this letter, Notice of Liability and Declaration of Status for an airline so that my family and I may travel freely.  

1) You will need to tailor the letter and Declaration of Status for you.

2) The Acknowledgement page needs to be printed for both the Notice of Liability and Declaration of Status.  Therefore, you will need to print it twice.  This is the form you take to get notarized by the De Facto.  Perhaps your bank offers free notaries as mine does.

3) The Notice of Liability is applicable to many circumstances, such as the school your off spring will be attending or for your employer.

To share my experience thus far, I sent mine out to United via registered mail and they sent it back unopened.  I will need to get clarity on what happened.
